ALL patients are at risk to fall. We want to identify those who are at high risk to fall. Our facilities use the Morse Fall Risk Screening.
The fall risk assessment should be done; On Admission, Every Shift, Transfer between units, change in caregivers, change in patient condition, and immediately after a fall.
Verify/change the time of the fall risk assessment to reflect the time the assessment was actually completed.
