Please review the below changes to Pressure Injury Admission Skin Risk Assessment updates.

 

The compliance calculation for admission skin risk assessments has changed.

 

Previously, all metric calculations for Pressure Injuries were based on two shifts: the day shift from 7 a.m. to 7 p.m. local time, and the night shift from 7 p.m. to 7 a.m. Under the previous rules, an admission skin risk assessment was required to be completed by the end of the patient's first shift after they were admitted (either 7 a.m. or 7 p.m.). 

 

This rule created fallouts for patients admitted close to the end of a nursing shift in which it is unrealistic for the nurse to perform the initial risk assessment. 

 

The 7 a.m./p.m. shift deadline has now changed for admission assessments. Instead, the assessment must now be completed within 12 hours of the patient’s admission. After that, a shift change assessment is required for the next shift and every shift onward.

 

This change is retroactive, meaning that the historical compliance scores for admission risk assessments have been recalculated using the new rules.
