PCT Behavioral Health Sitter Documentation

Hi Team, 

Below is required for all PCTs in regards to sitting with behavioral health patients. 

3. Q15 min documentation is completed and handed off
4. Break PSA/Tech/RN has to sign sheet & make sure to initial sheet during that break time.  
5.  If patient is listed as a risk the sheet documentation is to be started at that time - in ER please communicate with primary RN to ask time risk assessment is complete to ensure q15 min is started on time.
