Suicide Checklist and Documentation Policies

In partnership with Medical City Arlington, please refresh yourself with the suicide documentation requirements.

Please remember you only need to document suicide on admission if your patient:

1. Admitted for suicidal idealation or behavioral issues

2. Verbalizes or attempts self harm.

If you complete the suicide assessment and anything other than NO RISK is scored, you need to follow up and notify the physician.  This must also be documented in Manage/Refer/Notify.
