Summary

Reporting to the Chief Financial Officer, the Director of Case Management provides overall leadership and strategic planning for the Case Management and Social Services departments.

Responsibilities include:

• Overseeing department/program evaluation
• Ensure appropriate utilization of resources, facilities and services by reviewing admissions, duration of stay, professional services rendered for both inpatients and observation patients
• Develop departmental goals
• Create and implement action plans
• Identify patient discharge needs and work in collaboration with nurses and physicians in managing patient care need through the organization
• Supervise Case Managers

Qualifications

Requirements:

• Bachelor’s degree required
• Current RN license strongly preferred
• 3-5 years Case Management experience in an acute hospital setting
• 2+ years in a management role, Director level experience in Case Management strongly preferred

Compensation commensurate with experience with salary range between $80K (Min) - $100K (Max) plus Bonus potential
